Living with COVID PPE Bundles and PPE Emergency Provision Order Form for General Practices, GPRCs & ACCHSs

Please note: There are changes to ordering and delivery process

This order form is intended to supply approximately one month’s supply of PPE per practitioner for:

  • GPs, ACCHSs and GPRCs willing to manage COVID positive patients face to face, until 30 September 2022
  • GPs and GPRCs willing to support COVID positive people virtually, and respiratory patients face to face are eligible to access P2/N95 and eye protection, until 31 December 2022 (Please order under section 1a) 
The Commonwealth is commencing a more streamlined approach to Personal Protective Equipment (PPE) and pulse oximeter ordering, distribution and delivery.

  • PPE deliveries will be delivered directly from the National Medical Stockpile (NMS) to general     practitioners and immunising pharmacists  (rather than via PHN)


Section 1: Living with COVID PPE Bundles (per practitioner) contain:


What general practitioners are eligible for Living with COVID PPE Bundles?  


The following eligibility criteria will be applied to Living with COVID PPE Bundles:  

  • GPs, ACCHSs and GPRCs willing to manage COVID positive patients face-to-face, until 31 December 2022  
The Living with COVID PPE Bundle is intended to supply each practitioner with 1 month supply of PPE (equivalent to 40 patient visits). If practitioners have a higher case load (i.e., over 40 patients), bundles can be ordered more frequently.  

Section 1a: Living with COVID PPE- GPs and GPRCs willing to support COVID positive people virtually, and respiratory patients face-to face ordered per practitioner. 

Each bundle contains:
  • P2/N95 respirators (90 units) and either goggles or face shields (40 units, supplying each practitioner with 1 month's supply of PPE.
  • General practitioners must provide PHNs with the provider numbers of relevant general practitioners managing COVID positive patients, for whom the PPE is sought.

  • Please Note: General practitioners must provide PHNs with the provider numbers of relevant general practitioners managing COVID positive patients, for whom the PPE is sought. Provider numbers will be used by the Department of Health to conduct allocation and verification checks.  

Section 2: Living with COVID PPE Bundles ‘Top Up’ PPE order per practice:


These individual items can only be ordered in rare circumstances that stock has been damaged in transit i.e. broken bottle of hand sanitizer. Only one unit can be accessed.

Section 3: Living with COVID PPE Bundle Pulse Oximeters


Note: Practices can now order additional pulse oximeters if required to manage a higher case load of COVID positive patients.


Section 4: Emergency Provision PPE ordered per practice


The following eligibility criteria will be applied to Emergency Provision PPE:  

  • GPs and GPRCs willing to support COVID positive people virtually, and respiratory patients face to face are eligible to access P2/N95 and eye protection, until 31 December 2022 (Please order under Section 4 below)

  • Emergency Provision PPE can only be accessed where there is no local supply available commercially, or from another State or Territory government scheme.

Please Note:

We would also like to inform you the hand sanitiser is due to expire 10 October 2022, and the NMS (National Medical Stockpile) will not be ordering additional quantities of this item. Hand sanitiser will be included in bundles while stock lasts and then will be discontinued. Practices are encouraged to seek local supply available commercially for this PPE.

General Practice Details









Section 1: Living with COVID (LWC) PPE Bundles ordered per practitioner

The following eligibility criteria will be applied to Living with COVID PPE Bundles:  

What type and quantities of PPE are available in a Living with COVID PPE Bundle?  


The Living with COVID PPE bundles includes the following items, and are intended to provide 4 weeks supply of PPE. Each bundle contains:  

  • P2/N95 respirators (90 units) 
  • Either goggles or face shields (40 units) 
  • Full length gowns (40 units) 
  • Gloves (100 units) 
  • Surgical masks (80 units) 
  • Hand sanitiser (2 bottles)
Please fill out the below table for each eligible general practitioner  
  • This bundle is intended to supply each practitioner with 1 month’s supply of PPE (equivalent to 40 patient visits). If practitioners have a higher case load (i.e., over 40 patients), bundles can be ordered more frequently. Please complete a separate line for each practitioner indicating size choice. 
  • You have the option to select preferred size of P2/N95 respirators (small or regular) and gloves (small, medium or large). Please tick to indicate size.
  • The Department of Health will endeavor to meet your size choice, however in the event of stock unavailability, the Department will automatically allocate the next available size. 
  • General practitioners must provide PHNs with the provider numbers of relevant general practitioners managing COVID positive patients, for whom the PPE is sought.

Please Note: If a GP requires 2 or more LWC bundles per month, this GP must be willing to see more than 40 COVID positive patients per month.

  • P2/N95 Respirators (90 units)  
  • 1 Goggle Face Shield (1 unit)  
  • Gowns (40 units)  
  • Surgical masks (1 box)  
  • Hand Sanitiser (1 bottle)
