CESPHN Care Finder Program Community Consultation Survey

What is the Care Finder program?

The Care Finder program is a Commonwealth-funded aged care initiative. It is part of the governments response to the Royal Commission into Aged Care and Quality and Safety's recommendation for more localised and face-to-face supports to help connect people with relevant aged care services and supports.

Central and Eastern Sydney PHN (CESPHN) is aiming to support Care Finder organisations to provide assistance to people who have one or more reasons for requiring more intensive support to access aged care services.

Service delivery is expected to begin in January 2023.

About the survey

This survey will assist CESPHN identify opportunities for improving the Care Finder program and ensure the CESHN Care Finder program meets local needs in relation to care finder support. 

Community members, and health and aged care providers are invited to complete the survey.
